  • Anonymous Tip Form
  • If you have seen any kind of suspicious activity in Canby, please complete this form and we will receive your information via email. Providing your name and contact information is optional; however, please provide as much detailed information as you can. That way we can follow up on the manner as quickly and effectively as possible.Your cooperation is very important but do not take unnecessary risks or action on your own.

  • Traffic Complaint Form
  • This form can be used to assist you in making a complaint about various different traffic related issues. These issues can range from speeding, red light running, stop sign violations to traffic hazards, etc. Please complete the form and then select the method of return response before pressing the "Submit Query" button. All complaints will be addressed and a return response will be made as soon as possible.

  • Vacation Check Request
  • The Canby Police Department would be glad to check your home to see that it is secure while you are on vacation.

    The purpose of this form is to let us know, in the event of an emergency, that your home is vacant. It will also give us a resource to call if there is a problem at your home while you are gone. The information on the form will be placed on a log where officer's will be made aware that your home is vacant.

  • Citizens Police Academy
  • The Canby Police Citizen Academy provides a mechanism for educating the public about the criminal justice system, crime prevention, and a means to strengthen the citizen/police partnership to make the community a better and safer place to live. Prior to acceptance to an academy you must fill-out this online registration form and submit it to us, we will receive your information via email. For Canby residents only. It will be necessary that all individuals understand that a criminal history background check will be initiated on all applicants.

  • Alarm Permit Form

  • The City of Canby requires by ordinance that persons who have burglary or robbery alarms that summon police to any building or premises first obtain an alarm user permit. Alarm users who have more than two false alarms during the permit period (calendar year) are subject to a surcharge. Please complete the application and return it to the Police Department with a check or money order for $24, made payable to the CITY OF CANBY.

  • The Canby Police Department is continuously striving to provide the highest quality service by monitoring the performance of its employees. We encourage you to print the following form and use it to submit your complaint or commendation. The form must be printed and filled out completely and accurately.

  • Public Education Request Form

  • The Canby Police Department's Crime Prevention team provides many services to the community to encourage crime prevention and safety. If you are interested in having a program in your area and would like to request public education services on any crime prevention topic, please print and fill out the above form. Programs are scheduled based on the availability of personnel.

  • HomeSafe Application Form

  • Available to residents of Canby, the HomeSafe Program, is designed to aid emergency care providers in identifying where a displaced, memory impaired person may belong when s/he has wandered away from his/her home or care center. Identification is made possible by a specially designed bracelet provided at NO cost to the individual by the Canby Police Department. Please print and fill out the above form and bring it to the Police Department. Upon completion of the application, a volunteer collects the appropriate information to enroll the person in the program and takes a photograph.
